Please complete each section fully. Family information is entered once. Child sections can be added for every child you are enrolling.

Immunization records

Ages 0–25 months: provide an updated immunization record. Ages 3 and up: provide an updated record only if your child received vaccines in the past year.

Annual enrollment fee: $75 for a single-child family. The fee is due upon enrollment. Families who enroll in June, July, or August may have the new-year fee prorated.

We use Procare for parent communication and attendance. After enrollment you will receive an email with a 10-digit code. iOS · Android
